Accident and Sickness Limited Benefit Plan

                            

A goal of Albany College of Pharmacy and Health Sciences is to provide the best possible academic experience for all of our students.  As stewards of the school, the administration recognizes that the promotion of health and wellness is vital to your academic success. 

Both the College and its students have mutual investment that should be protected.  Adequate health coverage can serve to keep students in class and protect students and school against attrition due to the potential financial hardship caused by incurred medical expenses. 

ACPHS requires all full-time students to have health coverage, either through a family health insurance plan, a private insurer or a quality, low-cost plan offered by the College.  The school plan is serviced by Niagara National, Inc. and underwritten by Markel Insurance Company. 

Students must provide the name of their health insurance provider and their policy number in order to waive the College's Accident and Sickness Limited Benefit Plan.  If you do not provide this information, you will be enrolled in the College plan and your student account will be billed accordingly.

Registration/Waiver Procedures
Students may access the registration/waiver form online at
http://forms.acphs.edu.  Use your regular ACPHS Username and Password to login.  Then click on the Health Insurance Registration/Waiver Form.  The deadline for Registration/Waiver is August 1, 2009.  Please note that this request was sent to you electronically to your ACPHS e-mail account.  If you have completed this form for 2009-10, it is not necessary to complete it again.

International Students
In order
to waive the College's Markel Plan, International Students must complete the on-line waiver process described above and provide proof of coverage in the United States.  To provide this proof, International students are required to have their insurance provider submit a letter to the College.  This letter must:

  1. state that the student is covered in the U.S. under their insurance policy
  2. be on company letterhead
  3. state the student's name, permanent home address
  4. be signed by an insurance company administrator
  5. be mailed to the Office of Student Services by the Insurance Company.  Faxes and electronic submissions will not be accepted.
